Who can make a Referral
Anyone can initiate a hospice referral, including:
- Physicians and Nurse Practitioners
- Hospitals and Discharge Planners
- Skilled Nursing Facilities
- Assisted Living Communities
- Home Health Agencies
- Social Workers and Case Managers
- Family Members
- Patients
If hospice may be appropriate, simply contact our team. We will coordinate with the patient’s physician to determine eligibility and guide everyone through the admission process.

Clinical Review
Our nurses review the patient's medical information, collaborate with the attending physician, and determine hospice eligibility.
Family Consultation
We meet with the patient and family to explain hospice services, answer questions, and ensure they understand their care options.
Admission
Once eligibility is confirmed and consent is received, hospice services begin promptly—often the same day or within 24 hours when appropriate.
