Serenity Hospice is proud to be part of the Ocean Healthcare Network, an established New Jersey-based healthcare network providing an extensive continuum of healthcare services throughout New Jersey and surrounding areas.
Ocean Healthcare’s network extends far beyond hospice. Today, its continuum includes 12 skilled nursing facilities, hospice services reaching all 21 New Jersey counties, home health care, home-based nursing, short-term rehabilitation, long-term care, adult day health centers, assisted living communities, acute psychiatric hospitals, and outpatient behavioral health services.
This broad healthcare experience allows Ocean Healthcare Network members to support patients through many different stages and levels of care. Its skilled nursing facilities provide short-term rehabilitation, post-acute and long-term nursing care, while its community-based programs provide home health, skilled nursing, hospice, assisted living, adult day health, and behavioral healthcare.
What makes the Ocean Healthcare model especially meaningful is its focus on a seamless continuum of care. Network providers collaborate across different levels of care so that patients can transition between settings while maintaining better continuity, communication, and understanding of their individual healthcare needs.
